What to Bring

Please bring a valid photo ID, your insurance card, a list of current medications, and any referral letters from your primary care physician. Arrive 15 minutes early to complete registration.

Most insurance plans require a referral from your primary care provider. If you have prior test results or imaging, please share them with our office to avoid repeat procedures.
